Registration is easy! Begin by printing the Health Programs Registration form (pdf). This form may be used for one person or for a group by listing the class and the names of the individual participants.

• Mail – complete the registration for and send it with a check, money order or credit card information to:

• In-Person - come to the Continuing Education Office in the Curtice-Mott Building CM 1117. Office Hours are Monday – Thursday 8:00 am to 7:00 pm Friday 8:00 am to 5:00 pm

If you want MCC-CE to bill a third party for your course, submit a letter of authorization along with your registration form. The signed Authorization Letter, must be printed on company letterhead, include a purchase order number and each student's name and social security number. This letter must be received at least five (5) working days before the class begins. The college will bill the third party directly. Authorization to bill must be unconditional, and not contingent upon a student’s successful completion of the course. The student will be responsible for amounts unpaid by the sponsor.

Refund Policy
No refunds are given for Health Program seminars or courses unless the course is cancelled by the Department of Community Education.
