DisAbility Services will arrange reasonable and appropriate accommodations
for students with disabilities when the following qualifications
are met:
Documentation of the disability by a qualified professional
must be provided within 30 days of application for services. Documentation
cannot be more than five years old.
Student must identify him/herself and request services by signing
a Student Support Plan and agree to monitoring progress and service
by DisAbility Services staff.
Specific services included in the Student Support Plan are
prescribed based on a disability documentation received. The accommodations perscribed vary based on individual needs and disability in accordance with the written diagnosis. Documentation requirements vary based on the disability.
DisAbility Services does not provide personal attendant care
or transportation.
Student should be able to travel independently to and from
classes and campus buildings.
Student receiving in-class support must attend class on a
regular basis or risk losing those services.
Student may be required to select a designated section of
a course to receive interpreter services (due to the limited
number of interpreters).
Student must maintain contact with their assigned DisAbility Services Specialist at least three times during each semester
(the start of the semester, prior to mid-term, and prior to final)
to review progress and services.
Due to limited resources, DisAbility Services reserves the
right to prioritize distribution of equipment or services.
