Appendix A: Employment Forms

Appendix B: Tips For Job Success

Students are employed as trainees and should be guided, encouraged and provided with productive supervision. Please follow the instructions below when requesting student employees:
1.
Completely fill out the Student Employment Job Request Form available on Lotus Notes through the Student Employment Job Request database. Be sure to mark the classification of student employee wanted (either CWS, S.A., or both) along with the semester(s) that employment is required. One form is sufficient for Summer, Fall and Winter. A new Job Request Form is always required for the Spring Session. A separate Job Request Form with a complete job description is required for every different position within the department. For example, one form is sufficient for six different people as long as they will all be doing the same job. Remember, being as specific as possible, including days and times that coverage is needed, will enable an expedient selection from the student employee applicant pool.
2.
Submit the Student Employment Job Request Form with the number of hours requested to the appropriate cost center manager for their on-line signature approval by clicking on the submit button.
3.
The person completing the job request as well as the cost center manager and the staff in the Student Employment Center are able to view the progress of the job request. It is a good practice for the employer to keep a copy of the form for future reference.
4.
Please complete a new Student Employment Job Request Form when requesting additional hours or semesters.

Note: Non-compliance may result in departments forfeiting their rights to employee students.
